Physician-led hospital, post-acute, skilled nursing, and wound care — designed around continuity at every stage of the patient's journey across Michigan.
From hospital admission through post-acute recovery, CoreMed delivers physician-led, integrated care for Michigan's medically complex adult patients.
Physician-led medical management for hospitalized adult patients. CoreMed works collaboratively with hospital teams, specialists, nurses, patients, and families to coordinate treatment and provide patient-centered inpatient care throughout the hospital stay.
Medical management of patients experiencing acute illnesses or conditions requiring hospital-level evaluation, monitoring, and treatment. Our physicians ensure timely, coordinated care during critical episodes.
Comprehensive medical care for adult patients — including those with complex, multiple, and chronic medical conditions. Our internists bring deep diagnostic expertise and a whole-patient approach to every encounter.
Continued physician oversight following hospital discharge. CoreMed helps manage patients through the next stage of recovery while maintaining clear communication between facilities, physicians, patients, and families.
Physician-led medical management for patients recovering or receiving ongoing care within skilled nursing facilities. Our goal is to maintain continuity after hospitalization and properly manage the patient's medical needs throughout recovery.
Structured coordination between the hospital, primary care physician, specialists, skilled nursing or post-acute facility, patient, and family following discharge — reducing gaps and keeping every member of the care team aligned.
CoreMed also specializes in the evaluation, management, and treatment of acute, chronic, and difficult-to-heal wounds across appropriate care settings. Our wound care services are integrated into the broader continuum — so patients with complex wounds receive specialized attention without being transferred away from their care team. From initial assessment through recovery, we support healing at every stage.
